This test is used in the clinic to assess for a knee effusion, or swelling in the knee joint and will be positive when there is a certain amount of intra-articular fluid in the knee joint. You can see on the inside (medial to the patella) of the kneecap a positive “bulge” sign when the hand is brushed back down towards the patients feet, indicating that there is a decent amount of fluid in the joint.
